Description:
Transitional care needs of people with complex health and social needs, and those of family caregivers are often poorly managed. This course focuses on the knowledge needed to care for patients at particular moments across the continuum of care and through transitions in an illness experience. It addresses nursing phenomena of concern and common pathways of illness, with an emphasis on family-centered care through transitions in the illness and recovery phases. Transitional care includes a range of time-limited services designed to ensure health care continuity as individuals move from one level of care to another, among multiple health care team members, and across settings. Prerequisites: Full Major status in Nursing
